Can botox cause jowls or facial sagging? A straight answer from Cheongdam, Gangnam, Seoul

Botulinum toxin does not loosen skin. What it can do, when a dose is heavy or a point is misplaced, is remove muscle support or weaken a muscle that was holding a corner up — temporary effects that look like sagging. True laxity is a separate finding with separate treatment.

The Short Answer

What toxin can and cannot do to a sagging lower face

Skin laxity is a property of skin and the tissue under it, and an injection into a muscle does nothing to either. Three things can still make a face read looser after treatment: a chewing muscle reduced further than that face could spare, a dose that reached a muscle holding something up, and a finding that was never muscular in the first place. All three are dose-and-placement questions, and all three are temporary.

The word jowl describes soft tissue hanging along the jawline. It appears when the tissue that used to sit above the jawline loses its hold, which happens with age, weight change and sun exposure. Botulinum toxin type A reduces the nerve signal to a muscle for a while; it neither cuts the ligaments that hold tissue up nor changes the collagen in skin. The question is worth taking seriously anyway, because there is a real route from an injection to a face that looks looser, and pretending otherwise would not help you decide.

That route runs through the masseter. A chewing muscle is part of the lower face's structure, so reducing a large one softens an outline while reducing a modest one in an already lean face takes away support the cheek was using. The result reads as flatness or slackness along the lower cheek, and it fades as the muscle recovers. Our answer to it is the dose, and the reading that comes before the dose.

The Three Routes

How an injection ends up looking like a sagging jawline

Each of the three has a different fix, which is why naming the right one matters more than reassurance.

ROUTE 01

Masseter volume taken past what the face can spare

Heavy dosing, or repeated sessions stacked closely on a lean face, reduces chewing muscle beyond the point where the outline benefits. The lower cheek loses some of its underlying fullness and reads softer. It recovers as the muscle rebuilds, and it is avoided by dosing low and reviewing before adding.

ROUTE 02

A point that reached the wrong muscle

Small muscles around the mouth and along the lower face lift and shape a smile. Product that spreads into one of them, or a mark placed outside the working area of the masseter, can leave a corner sitting lower or a smile pulling unevenly for several weeks until the effect wears off.

ROUTE 03

Laxity that was mistaken for a muscle problem

Where the jawline was already losing definition, softening a muscle underneath makes the looseness more visible without having caused it. This is a diagnosis failure and not a drug effect, and it is the reason the reading is done before a vial is opened.

Ordinary risks with this molecule belong on the same page as the rare ones: bruising at an entry point, brief swelling, mild asymmetry that needs evening up, a heavy-feeling lid when a brow dose runs high, and dimpling of the skin over the jaw angle when product is laid too superficially. Every one of those is temporary. None of them changes the structure of your face permanently, and all of them are less likely at a dose set for the muscle actually in front of the needle.

How Dear Doses

The planning rules that keep a lower face supported

Dose and placement are the treatment with botulinum toxin, so both are decided by the director who examined you, from a reading of your own face. First sessions are set deliberately light, the response is assessed at a review, and units are added only where the muscle asks for them.

Published rates are the same regardless of how cautious the plan is: ₩40,000 (≈ $30) per session for the chewing muscle on the Korean tier, ₩350,000 (≈ $250) on the imported one, and ₩30,000 (≈ $20) for a single expression area. Restraint costs you nothing here, and the full card is on the price list.

If You Are Worried

What to do if your face looks looser after an injection

Book a review and have the cause named before anything else is injected. Weakness from a misplaced point recovers on its own as the effect wears off, and a masseter rebuilds under normal chewing. Treating a temporary change with more product is how a small problem becomes a longer one.
Dear, Knowhow

Why this question is answered before an injection here

01 — THE FULL ARSENAL

Laxity has its own room, so a toxin plan can be refused

Lifting devices and threads sit on the same floor as the injectables.

02 — DIAGNOSIS FIRST

EVE Muse 3D + AI separates loose tissue from muscle bulk

Three-dimensional imaging under five light sources is read alongside an examination of how the lower face behaves when you clench, smile and rest. That pairing catches the case where a jawline is losing its hold and a relaxant would only make it more visible, which is the single most useful thing a scan does before this treatment.

03 — DIRECTOR-PERFORMED

The person who read your face sets the units

Injections are performed by your director, who marked the points, and a review is booked before you leave. Depth and position decide whether skin over the jaw angle dimples and whether two sides soften evenly, so the cautious first dose is a house rule and the review is where any unevenness is corrected.

FAQ

Toxin and a loosening jawline, question by question

Does botulinum toxin damage or stretch the skin?
No. The molecule acts at the junction between nerve and muscle, and it has no mechanism for changing collagen, elastin or the ligaments that hold facial tissue in place. Loosening of skin comes from ageing, sun exposure, weight change and genetics. What an injection can alter is how hard a muscle underneath is pulling, which is a different thing and a temporary one.
Can masseter treatment make my cheeks look flat?
It can, when the muscle is reduced further than that particular face had to give. A chewing muscle adds fullness along the lower cheek as well as width at the angle, so on a lean face a heavy dose can read as flatness. That is why the first session here is dosed low and reviewed, and why we sometimes recommend leaving a modest masseter alone.
Is the change permanent if it happens?
No. The nerve signal recovers, the muscle returns to its former strength and bulk rebuilds under normal chewing. Weakness in a misplaced muscle fades on the same kind of timeline. What does not reverse on its own is true laxity, which is why separating the two at the examination matters more than any reassurance about the product.
Why does my jawline look less defined after a session?
Three possibilities, and your director can tell them apart by hand. Reduced muscle volume changes the shape under the skin; a point that reached a neighbouring muscle changes how a corner sits; and tissue that was already loose becomes more visible once the muscle under it softens. The first two settle with time, the third is assessed as a lifting question.
Can wrinkle treatment cause sagging higher up the face?
Sagging, no. A heavy or badly placed dose near the brow can leave a lid or brow feeling heavy for a few weeks, which is often described as drooping. It comes from relaxing a muscle that was lifting, and it wears off. One expression area is listed at ₩30,000 (≈ $20) on the Korean tier, and a first plan there is deliberately light for exactly this reason.
I already have some jowling. Should I avoid masseter treatment?
Not necessarily, but it changes the conversation. Where the width is genuinely muscular and the laxity is mild, a lighter dose can still suit you. Where the jawline is losing its hold, reducing the muscle under it makes the looseness easier to see, so we do not recommend the injection and the assessment moves to the lifting category instead.
Does having toxin regularly over years cause sagging later?
There is no established mechanism by which a temporary relaxant loosens skin, and the references below describe the molecule's action and product properties without reporting that outcome. What long-term masseter dosing does influence is muscle bulk, since a muscle kept working at reduced force stays smaller. Your director tracks that across sessions and adjusts or pauses a plan accordingly.
What does a dose that is too high actually look like?
On the lower face, flatness along the cheek and an outline that has lost some of its structure. Around the mouth, a smile that pulls unevenly or a corner sitting lower than the other. At the brow, a heavy lid. All three are visible within the first weeks, all three wear off, and all three are less likely when units are added at a review instead of guessed at the first visit.
Would you tell me if you thought the injection was a bad idea?
Yes, and it is a normal outcome of the diagnosis here. Width held in bone, fullness that is glandular, hollowing from volume loss and tissue that has lost its snap are all findings a relaxant cannot help. In each case we do not recommend the injection, you are shown the finding on your own scan, and the treatment that would reach it is named.
How soon can I be seen if something looks wrong?
Quickly. A review appointment is written into the plan when the injections are booked, and Dear Clinic is open 365 days a year including weekends and public holidays, so an earlier visit can usually be arranged. Bring the photographs you have. Nothing further is injected until the cause of what you are seeing has been named.
What is treated at Dear when the finding is laxity instead of muscle?
The lifting category, chosen on the examination: focused ultrasound, radiofrequency tightening, threads, or a combination planned across sessions. Published rates for each sit on the price list, and the choice is made from depth and tissue quality on your own reading. Square jaw treatment remains listed at ₩40,000 (≈ $30) per session for the cases where the muscle genuinely is the cause.

The three papers below describe the clinical action of botulinum toxin type A in aesthetic use and the laboratory differences between licensed products. They are cited here for the mechanism and the dosing logic that follows from it.

None of them is a study of lower-face sagging, and nothing on this page should be read as a finding from them about your own jawline. The examination is what decides that.
