Mechanical Lift vs Energy Lift

Thread Lift vs HIFU / RF

Threads move tissue that has already dropped. HIFU and RF rebuild collagen where it has thinned. The two answer different complaints, are sold in different units — per thread against per shot — and often end up on the same plan.

Short Answer

What threads do that energy cannot

A thread lift is mechanical. Fine PDO or PCL threads go in through a cannula and carry dropped tissue back up, and the change is there when you sit up from the couch. HIFU and RF are energy. Focused ultrasound reaches the SMAS at 4.5 mm and radiofrequency heats the dermis, and both work by making your own collagen contract and rebuild over the following weeks. A defined jowl is a placement problem. A whole lower face that reads slack is a collagen problem.

The units follow the mechanism. Threads are quoted one at a time, from ₩100,000 for a PDO or PCL thread, because the count depends on how far the tissue has to travel. Energy is quoted in blocks of shots, from ₩150,000 for 300 Shurink Universe shots, because coverage is what you are buying. Your EVE Muse 3D + AI diagnosis measures both the descent and the skin thickness before either number is written down.

Threads are quoted individually: ₩100,000 for a PDO or PCL thread and ₩400,000 for a Silhouette Soft thread, with the count set at consultation. Energy sessions are quoted per shot block — Ultherapy Prime at ₩400,000 per 100 shots, Thermage FLX at ₩2,200,000 for 600. One list, one price, whoever is reading it.

By Your Case

Where each of the three earns its place

Three complaints, three answers, in the order they usually arrive.

CASE 01 — THREAD LIFT

Clear, localized sagging

A jowl that has visibly dropped is a placement problem. Threads are quoted per thread — ₩100,000 for PDO or PCL, ₩400,000 for Silhouette Soft — and the count comes from how far the tissue has to be carried. You see the change the moment you sit up.

CASE 02 — HIFU

Overall laxity, no needles

When the whole lower face reads slack and you want no puncture before an event, focused ultrasound reaches the SMAS at 4.5 mm and leaves the surface untouched. Expect little in the first fortnight; the collagen answer builds over the weeks that follow.

CASE 03 — RF

Texture & firmness first

Crepe texture, thinning and lost bounce all sit in the dermis. Radiofrequency heats that layer broadly and refines the surface. It will not move a jowl.

The Combination

Why they're often prescribed together

Most faces past forty present both problems at once: a dermis that has thinned and a jaw outline that has moved. A common Dear protocol lays the energy foundation first and adds structure where it is still missing.

The energy session goes first, so the tissue the threads will carry is already firmer.
Threads follow, placed only where the outline has actually moved.
The energy result matures over months while the thread lift shows immediately — the timelines complement each other.
The interval between the two is set so no area is asked to heal at two depths at once.

HIFU (Ultherapy Prime, Shurink Universe), RF (Thermage FLX, Density, XERF, Onda, Titanium) and thread lifting are all on the premises, so the mix and the interval are decided once, at the diagnosis, and booked in one go.

Dear, Knowhow

How the three get sequenced here

01 — WHAT IS ON THE FLOOR

Threads, HIFU and RF on one floor

Thread lifting, Ultherapy Prime, Shurink Universe, Thermage FLX, Density, XERF, Onda and Titanium are all here. Moving your plan from one of them to another is a change of treatment room.

02 — THE SCAN

Skin thickness before thread count

The EVE Muse 3D scan and its five light sources read where the descent sits and how thick the dermis is over it. Thin skin above a dropped jowl changes the thread type, the entry point and the depth the energy is set to.

03 — SEQUENCE AND DEPTH

One plan, precise hands

Bruising, swelling, a palpable thread end and a dimple at the entry point are the risks that careless sequencing produces. Your plan is designed and performed by our directors, and the intervals are theirs to set.

When We Say No

When a lift is the wrong answer

Some faces are not sagging at all. What reads as "sagging" is sometimes volume loss better addressed with filler or a skin booster, and a thread pulled through a hollow cheek only sharpens the hollow. When the scan points there, we decline the lift and quote the filler or booster instead. The commonest mistake made with this group of treatments is treating a face that needed one small correction, and having every device on the floor is what makes stopping at one possible.

Caution

After threads, after energy

Threads: swelling and bruising along the entry points for a few days, and a pulling sensation when you open wide.
Threads: keep chewing gentle and sleep on your back while the tissue settles.
Energy: redness and a warm, taut feeling that fades within a day.
Either way, no sauna and no alcohol for a few days.
A visible dimple, a thread end you can feel, or pain that grows instead of fading is worth a same-day call.
The follow-up call and the aftercare visit are already inside the fee.

FAQ

Thread lift vs HIFU/RF, answered

Is a thread lift or HIFU the answer for a dropped jawline?
It depends whether the outline has moved or only softened. A jowl that has physically descended is a placement problem, and threads carry it back where it was, visibly, the same afternoon. A jaw that is merely losing definition is a collagen problem, and focused ultrasound at the SMAS works better on it. Many jawlines show both, and then the energy session goes first.
Both HIFU and RF are energy — what separates them?
Depth and spread. HIFU focuses ultrasound into small columns at set depths of 1.5, 3.0 and 4.5 mm, so it can reach the SMAS, the layer a surgical lift moves. Radiofrequency heats the dermis broadly and does not go that deep, which makes it the better instrument for crepe texture and lost firmness. Dear runs Ultherapy Prime and Shurink Universe for the first job, Thermage FLX and Density for the second.
How many threads will I need, and what do they cost?
Threads are priced one at a time: ₩100,000 for a PDO or PCL thread and ₩400,000 for a Silhouette Soft thread. The count comes from how far the tissue has to be carried and over how wide an area, and it is fixed on the written plan before the local anaesthetic goes in.
Do the threads dissolve, or stay in my face?
PDO and PCL threads are absorbable. They break down over months while the fine collagen track laid along their path keeps some of the support after the material has gone. Nothing permanent is implanted. Your director chooses the thread type and the vector from the treatment area and the lift you want.
Can threads and an energy device go into the same plan?
Yes, and the order is deliberate. The energy session goes first so that the tissue the threads will carry is already firmer, and the threads follow only where the outline has actually moved. The interval between the two is set by your director so no area heals at two depths at once.
Which one hurts, and which one bruises?
A thread lift is done under local anaesthetic, so you feel pressure and pulling more than sharp pain, and bruising or swelling around the entry points for a few days is normal. HIFU is felt as heat and a deep prickling as the energy lands in its columns, with a topical numbing beforehand and essentially nothing to hide afterwards. RF sits between the two in sensation and leaves the least trace.
What does each option cost at Dear Clinic?
Threads start at ₩100,000 each. On the energy side, Shurink Universe is ₩150,000 for 300 shots and Ultherapy Prime is ₩400,000 per 100 shots; on the RF side Density opens at ₩700,000 for 300 shots and Thermage FLX is ₩2,200,000 for its 600-shot tip. Every figure is on the published list.
How long does each of them hold?
A thread lift is commonly quoted at a year to eighteen months depending on the thread type, and energy results are usually reviewed at about a year. Both figures move with your age, your skin and how much was placed, so treat them as planning numbers and let the follow-up decide. Because the mechanisms differ, the two are maintained on separate schedules.
When would you refuse to do any of them?
When the complaint is volume rather than descent. A cheek that has emptied reads as a droop in the mirror, and a thread pulled through it sharpens the hollow instead of correcting it. In that case we decline the lift and quote filler or a skin booster. We also decline a same-week thread lift before a wedding or a flight, because the bruising window is not worth the risk.
How soon after a thread lift can I fly?
Swelling and bruising peak in the first two or three days, so a flight on the day after placement is unpleasant but safe enough. Visitors on a short trip more often take the energy session, which leaves nothing visible, and book threads for a return visit. Your director will say plainly which fits the dates you have.
Can this be done on the same day as the consultation?
An energy session usually can: the EVE Muse scan and the doctor consultation take about half an hour, and treatment follows once suitability is confirmed. A thread lift needs the consent conversation and the numbing time on top, so allow longer in the day. Dear Clinic is open 365 days a year, weekends and public holidays included.
Do visiting patients pay the same per-thread price?
Yes. The per-thread and per-shot figures on this page are the ones quoted at the desk, and the thread count and shot count are written on the plan you approve before anything begins. Nothing is added afterwards.
