A gummy smile is a question of where the toxin goes and how little of it is used. The lip elevators sit a few millimetres apart in a small, expressive area, so the appointment starts by measuring how your own smile moves before anything is marked on the skin.
Dear Clinic lists this work on its special-area toxin line, the same published tier that covers the salivary gland, the nostril area, the temple and the philtrum. Those zones share one requirement: a tiny dose placed exactly, in anatomy where a couple of millimetres of drift produces a different face. The rate reflects the mapping and the anatomy involved, and it is on the price card before you arrive.
Botulinum toxin type A works by interrupting the signal that tells a muscle to contract, which is why the change is temporary and why dose is a clinical judgement instead of a menu item. The pharmacology of the type A products in aesthetic use has been described in the peer-reviewed literature cited at the foot of this page, and the general principle has been in print since the first published series on glabellar lines in 1992.
What that literature does not contain is your smile. How high your lip rises, whether it rises evenly on both sides and how much of the visible gum comes from muscle at all are questions answered in the room, with the scan on screen.
Four small muscles share the work of raising the upper lip. Which of them dominates your smile changes where a dose belongs and how large it can safely be.
| Muscle | What it pulls | Why it matters for this injection |
|---|---|---|
| Levator labii superioris alaeque nasi | Runs from the side of the nose to the lip and the nostril wing, lifting both | The usual dominant elevator in a gummy smile, and the one whose over-treatment flattens the lift of the nostril as well as the lip |
| Levator labii superioris | Raises the central part of the upper lip | Placement here changes the middle of the smile line; too much and the lip reads long and inexpressive |
| Zygomaticus minor | Draws the upper lip upward and outward toward the cheekbone | Carries the outer smile. Treating it heavily is what produces a flat or lopsided grin |
| Depressor septi nasi | Pulls the nasal tip down as the lip moves | Relevant when the nose drops visibly on smiling; assessed at the same examination, treated only if the examination calls for it |
A gummy smile is rarely symmetrical. One side commonly lifts higher than the other, and the correction has to follow that difference instead of splitting a dose down the middle.
Three-dimensional capture under five light sources, with AI analysis, gives the director a baseline of the mid-face before the muscles are palpated. The scan is taken first for a practical reason: the discussion about what is achievable happens with measurements on screen instead of adjectives.
Gum show at a polite smile and gum show at a laugh are different measurements, and people are treated for the one they see in photographs. The director records how the lip behaves in both, notes which side climbs higher, and checks whether the nostril or the nasal tip moves with it.
An upper lip that lifts hard is one cause of visible gum. Short clinical crowns, gum tissue that sits low over the teeth and the vertical proportion of the upper jaw are others, and none of them answer to a muscle relaxant. This step is where a plan gets written, or where the examination sends you to a dentist instead.
Where the examination supports treatment, the director marks the points on your own anatomy and starts low. The number of units and the exact points are set in your consultation and written into your record; this page publishes no dose, because a dose copied from an article is somebody else's face.
Several of the causes of visible gum are structural. A muscle relaxant has no effect on any of them, and treating anyway spends money without changing the photograph.
Where the cause is dental or skeletal, we do not recommend the injection and the consultation ends with that written down. Related toxin work that is performed here — salivary gland placement for a wide lower face, masseter dosing for a square jaw — targets different tissue and fails in different ways.
Risks named plainly. Small bruises at entry points are the common event, and a headache after the appointment is reported. The specific downside of this area is a lip that feels heavy or reads long, a smile that becomes uneven if one side takes more effect than the other, and short-lived awkwardness with some consonants, whistling or drinking through a straw. These are consequences of dose and placement, which is why the first appointment is deliberately conservative and why a review is offered before anything is added. Individual results vary.
| Special-area line | Toxin tier | Listed rate |
|---|---|---|
| Gummy smile placement | Korean botulinum toxin | ₩60,000 (≈ $45) |
| Gummy smile placement | Premium imported botulinum toxin | ₩400,000 (≈ $285) |
The two rows differ by product tier, and which tier suits you is a conversation at the consultation, not a default. The rate is for the placement, the mapping and the fact that a director performs it, so it does not move with how expressive your smile turns out to be. If a second area is planned at the same visit, that area is quoted on its own line and the whole total is itemised before you agree. Published rates as of September 2026. Every figure Dear Clinic holds is on the transparent price list.
Wrinkle areas, masseter, skin toxin, salivary gland, hyperhidrosis and the special-area line all sit on the same price list in this building. A clinic that only performs the treatment you asked about has one answer available. Here the gummy smile line is simply the one that fits your examination, and the others stay where they are.
Five light sources and three-dimensional capture are pointed at the mid-face before any figure is quoted, and the reading is discussed with you on screen. For this treatment the decisive finding is usually the left-to-right difference in how the lip climbs, because that is what a symmetrical dose would get wrong.
Marking and injecting stay in one pair of hands, and the same director sees you at review.

I've visited clinic for LDM procedure. I don't speak Korean, so there was an interpreter who assisted me while consulting. The staff was very attentive, they asked me every time before applying products/covering my face during the procedure and it went smoothly. Planning to visit the clini…
All the staff are kind, the doctor is professional. They recommended us the treatment that fit us best and last the longest. A very nice experience overall.
I did a hydrofacial, white/black head removal, and 1 day lasers. Really kind staff and reasonable prices. The only thing I would advise is that your first consultation is with a salesperson and you don’t see the doctor until after you decide what you want and after you pay.
Had a great experience here as a foreigner! The doctors and staff were knowledgeable, friendly, and not pushy. They made recommendations in my consultation and were happy to work within my budget.
A full smile, taken straight on — that one image tells us more about whether this treatment fits than any description can.
No trial is cited here for a gummy smile dose, because this page publishes no dose. What the literature below establishes is the material the treatment rests on: the first published aesthetic series with botulinum toxin type A, a consensus review of its use in aesthetic practice, and a description of how the type A products in clinical use differ pharmaceutically from one another.
Each entry resolves to a DOI in an indexed journal. The judgement applied to your own smile is clinical and is made in person at your consultation.
